§ If you are in need of hospitalization, call and notify member services as soon as possible at 1-866-723-7232. A Case Manager will work with you and the hospital to negotiate a reduced fee for your procedure
§ Case Manager provides the following documentation to Member:
§ How the program works
§ Authorization Form for the release of information
§ Limited Power of Attorney
§ Release of Liability
§ Member signs and returns documentation and a case file is opened
§ Case Manager will assist in locating physicians if requested
§ Case Manager contacts the physician to obtain diagnosis, procedure codes and information regarding which hospitals the physician has admitting privileges to at which they can perform the procedure
